Forms for the General CMANA Membership

The following forms are available (there is NO online payment method the forms need to be down loaded filled out and returned to the address listed on the forms):

Brief Membership Application Form. This form is a two-page version of the standard application form. Historical information has been omitted.
Standard Membership Application Form. This form is the standard application form for membership in the CMANA. It contains the application, a pedigree tree for your genealogy and historical information on both Clan Maclachlan and the CMANA.

Forms for Convening

The following forms are available:

Expense Form. Use this form to submit for reimbursement for an allowable convening expense. Reimbursement is subject to prior approval.
Guest Registry. Use this form to sign-up visitors to your tent.
